Essential Functions:
· Under the close supervision and direction of immediate supervisor:
· Monitors and arranges for substitute care for children who have been committed to the long term care of the agency.
· Develops case plans and arranges for necessary and appropriate services for children, parents, and/or substitute caregivers.
· Arranges for and monitors the independent living skills training for children age 16 and above and placed in substitute care.
· Writes and completes court summaries and reports, correspondence, case histories, and other documents.
· Arranges for and conducts case reviews/staffings.
· Participates in court hearings and administrative reviews.
· Maintains organized, current case files.
· Counsels children, parents, and/or substitute providers.
· Attends workshops and training sessions. Responsible to serve on-call in agency’s 24 -hour emergency and crisis services to families and/or children in need. Perform duties under direction of on-call supervisor. Perform other duties as assigned by supervisor or administrator.
Required Qualifications:
· Requires at least completion of course work for Bachelor’s degree (social work, sociology, psychology, home training), as required by college or university and at least twelve (12) months paid experience. Prefer completion of course work for Master’s degree in subjects outlines above.
· Honesty, highest personal integrity with demonstrated attention to detail.
· Ability to establish effective working relationships with staff, administration and other professionals.
