Part-time Multisystem Youth Program Coordinator

The Champaign County Family and Children First Council is accepting resumes for the position of Multisystem Youth Program Coordinator.

MINIMUM QUALIFICATIONS:
Associate degree in social work or related field; OR equal number of college credit hours in a related field; OR two years’ experience facilitating team meetings using a Wraparound approach.
Bachelor’s Degree preferred.
Two years’ experience working with families, children and community organizations preferred.

DUTIES:
This position acts as the Child & Family Team Facilitator for multisystem youth and their family.
The Program Coordinator is responsible for completing a youth/family CANS assessment, building a family team with all key stakeholders, developing a Plan of Care and ensuring accurate documentation for every family enrolled in Service Coordination. The Program Coordinator is also responsible for monthly reporting to the State and local FCFC and assisting the Director with the management of program operations and financial management of the various programs of Council.

SALARY/PAY RANGE:
$18.00 – $22.00 per hour, based upon experience
